Navigator Program
The Navigator Program is a collaborative effort between Community Action House, The City of Holland, Evergreen Commons, 211 and Good Samaritan Ministries. The goal of the Community Navigator Program is to assure that individuals and families who are living in poverty and who need community services receive appropriate referrals and the assistance necessary to actually access these services.
The City of Holland will identify eligible participants and refer to 211. Case management services will be provided by Evergreen Commons and Community Action House, depending upon the age of the household members. Good Samaritan Ministries will link these participants to neighbors in their community for additional support and assistance. By collaborating together, the Navigator Program will build upon existing community resources and expand community linkages.
