Case Management Services are designed to meet client's needs that are not being met elsewhere.  Community Action House provides both long-term, multi-needs case management and counseling as well as short-term case management for clients with a single concern.

 

Emergency Case Management

The specific goal of the Emergency Case Management program is to help individuals and families better understand and more effectively respond to their immediate emotional, physical, or financial crisis. The aim is that each client will reach their personal potential and become successful members of our community.

Community Housing Partnership

The Housing Partnership's single focus is to prevent homelssness.  Families and individuals residing in Ottawa county, who meet the federal-HUD definition of homelessness, are provided with stable housing for up to two years.  During that time, Community Action House works alongside the family or individual to provide substantial support services.  Therefore upon completion of the program participants are able to secure and maintain permanent, stable housing and be financially self-sufficient.

Some of the services provided through case management are:

Finance & Budget Counseling
Case Advocacy
Counseling
Employment Training and Job Search
House Finding & Housing assistance
Transportation
Language Interpreting
Group activities
Service referral and follow-up
Home visits

Some of the clients who have been through our case management program have gone from homelessness to becoming home owners over a two year period. Community Action House is grateful for the privilege of helping our clients to become successful.

Life Skills Class

The Life Skills Class is designed to offer all of the components of our Case Management program in a structured group setting. The class consists of six two-hour sessions. Life assessment, goal setting, resource development, budgeting, employment and education, and home ownership are all class topics. Enrollment can be initiated with a self or agency referral and Case Managers will follow up with the individual.
